Why and how to use Mindable?

Mindable is an app for the treatment of panic disorder and agoraphobia (claustrophobia). Due to the anxiety disorder, everyday life and quality of life can be enormously limited. Quick help is needed, but often not possible due to long waiting times for a therapy place. The Mindable app offers immediate help here. In three treatment steps you learn to understand your fears, to trust your body again and to go through life independent of fear. The intuitive design of the app allows you to use it independently. However, you can also use the app in the context of psychotherapy together with a therapist, or give your family doctor insight into symptom progression and treatment progress.
You suffer from panic attacks and feel uncomfortable in crowds, narrow/wide spaces, or public transportation? You want to start treatment, but can’t find a therapy place or are on the waiting list? Then Mindable is just right for you.
Your doctor or psychotherapist can now prescribe Mindable for you with a confirmed diagnosis of agoraphobia (F40.0) and/or panic disorder (F41.0).
Mindable is not for you if you have been diagnosed with or are suspected of having any of the following mental disorders:

The term “DiGA” stands for “digital health application” (digitale Gesundheitsanwendung). These are CE-marked Class I or IIa medical devices whose main function is based on digital technologies. This main function is to assist in the detection, monitoring, mitigation or treatment of disease or detection, mitigation, treatment or compensation of injury or disability through digital technologies. A DiGA are thus a “digital assistant” in the hands of the patient.
Mindable is also a DiGA and therefore listed in the official DiGA directory of the BfArM. Psychotherapists and doctors can prescribe Mindable by prescription. The costs are covered by the statutory health insurance.
A so-called health or wellness app can be any available digital application on the international market. Manufacturers can offer these freely and are not subject to any review or monitoring.
A digital health application (DiGA), on the other hand, is a selected app or web-based application that may only be offered on the German market after a strict government review process. They are tested, according to German law, by the Federal Institute for Drugs and Medical Devices (BfArM). In addition, DiGA are Class I MDD medical devices and are also subject to strict quality and safety requirements under the Medical Devices Act. Furthermore, only DiGAs can be prescribed on prescription by physicians and psychotherapists and can be billed via the statutory health insurance funds.
Your doctor or psychotherapist can prescribe Mindable for you. Alternatively, you can ask for a doctor’s letter (Arztbrief) with proof of your diagnosis.
If you already have proof of your diagnosis, you can also send your request directly to the health insurance company.
Yes. The statutory health insurance companies cover the costs for Mindable in full. You do not have to make any advance or co-payments.
People with private health insurance can apply for reimbursement. Many private health insurances (PKV) also cover the costs for DiGA. However, there is no uniform regulation and it is up to your health insurance company to decide whether it will cover the costs for Mindable. It is best to ask your health insurance company directly.
If you have the okay from your health insurance, you can first purchase access to the app as a self-payer and then have it reimbursed retroactively.

After successful registration, you will receive access to Mindable’s treatment program. This includes:
As its main feature, Mindable offers three treatment modules that build on each other:
1. Learning & Understanding: Learn all about the mechanisms of action of your fears so you can get to know and understand them better.
2. Get used to symptoms: Through numerous exercises and animations, you will be supported in getting your body used to panic symptoms.
3. Become independent again: Overcome your fear by confronting anxiety-provoking situations and places. Mindable guides you through this process.
In addition, the app includes a diary function and a weekly checkup to document changes in your symptoms. The app summarizes your entries and evaluates them statistically for you. This means you always have an overview of your treatment progress. If necessary, you can share the evaluations with therapists and doctors via PDF export.
No. Mindable is not a substitute for a psychotherapist or doctor. We recommend to use Mindable as an accompaniment and support to therapy. Mindable is also suitable to bridge the waiting time for a therapy place and to counteract a worsening of the symptoms. The confrontation with anxiety-provoking situations and places requires (at first) a great deal of overcoming, we guide and accompany you in this process. However, your self-motivation is an important prerequisite for success.
No. The prescription of Mindable and traditional psychotherapy are independent health insurance benefits. Your eligibility for psychotherapy is not affected.
Mindable is based on cognitive behavioral therapy (CBT) methods with a focus on confrontation techniques. This is considered the scientifically proven gold standard in the treatment of agoraphobia and panic disorder. It is based on the treatment manual “Exposure-based therapy for panic disorder with agoraphobia” (Lang et al., 2018). The app was developed in close collaboration with the authors.
Currently, clinical trials are being conducted on the use of Mindable. Preliminary studies already show significant improvements in clinical symptoms, quality of life, limitations in daily life, and self-efficacy in Mindable users.
Mindable a Class I MDD medical device. We are therefore compliant with the strict quality and safety requirements of the Medical Devices Act. In addition, Mindable has been reviewed by the German Federal Institute for Drugs and Medical Devices (BfArM) and provisionally included in the DiGA directory. More information can be found here.
No. Mindable explicitly does not see itself as a “panic button”. Mindable therefore does not offer any functions to counteract an acute panic attack. It is scientifically proven that so-called safety and avoidance behaviors perpetuate anxiety and panic. You can learn more about this in the app.
Mindable supports you through targeted confrontation with anxiety and panic to get your body used to anxiety symptoms step by step, in order to reduce them in the long term.
In principle, Mindable can be combined with taking medication. However, it is important that you do not take any medication before and during the interactive exercises (symptom provocations, confrontations, etc.), which serve to calm you down. In the exercises it is explicitly desired and necessary that you feel fear, so that you can get used to them. You can find out more about this in the app.

Yes, Mindable creates statistical analyses of symptom progression and exercises. With your explicit consent, you can share this data digitally via PDF export or as a printout. Psychotherapists and doctors can get important information for your further treatment.
DiGA must ensure interoperability, i.e. the export of data according to common standards in the healthcare system in accordance with the DVG. And then, of course, we at Mindable do the same.
That’s why you can export your data (patient profile and observation data) both as a PDF, and in machine-readable form as a JSON file (according to FHIR standard). The machine-readable data is exported as a FHIR bundle (http://hl7.org/fhir/StructureDefinition/Bundle). The bundle contains a patient profile (http://hl7.org/fhir/StructureDefinition/Patient) and observation data (http://hl7.org/fhir/StructureDefinition/Observation).
You can find the data export in your profile within the app.
You can delete your account independently within the settings of the app. Your data will be deleted from our servers within the next 10 days. Your right to data portability according to Art.20 DSGVO will expire.
The Mindable Tech Tip:
Most problems can be solved by logging out and logging back in. The Mindable app goes through a reset each time. However, this does not mean that previous entries are deleted, they are of course retained.

No. Mindable is designed to be need-based, user-friendly, and intuitive. Technical knowledge beyond the everyday operation of a smartphone and apps in general, is not required.
To use Mindable, a working smartphone or tablet with internet access is required.
The Mindable app can be used with all smartphones/tablets that have the operating systems iOS 12 and greater, and Android 6 (API Level 23) and greater.
A valid email address is required to create an account. This does not have to include a clear name and identity.
A stable internet connection is only required during account creation and login. All other functions are also available offline.
The digital health application “Mindable: Panic Disorder and Agoraphobia” is aimed at people aged 18 and over who suffer from the symptoms of agoraphobia and/or panic disorder. The treatment program is based, according to S3 guidelines for the treatment of anxiety disorders, on methods of cognitive behavioral therapy with an emphasis on confrontation techniques.
Mindable aims to support users in addressing their anxiety proactively. Therefore, the treatment modules consist of psychoeducation, habituation of body symptoms through targeted symptom provocation, and in-vivo confrontation exercises.
In addition, Mindable enables the systematic recording of symptoms, anxiety trajectories and behavior. Treatment progress can thus be optimally documented and, if necessary, shared with treating physicians and psychotherapists.
Mindable is aimed at patients 18 years of age and older with a confirmed diagnosis of a
Agoraphobia (ICD-10 F40.0) and/or panic disorder (ICD-10 F41.0).

Mindable is prescribed on a panel prescription (Muster 16).*
The prescription must contain the PZN 17454202 and the name Mindable. Diagnosis and prescription duration do not have to be indicated on the prescription.
If you print prescriptions via the practice management system (PVS), you can find Mindable via the PZN: 17454202.
Alternatively, you can also issue a doctor’s letter (Arztbrief) with a confirmed diagnosis. Simply use our GKV template and our PKV template.

Your patient submits the prescription/diagnosis certificate to their respective health insurance company. This can be done online, via the health insurance app, by mail or by phone.
Important: Not in the pharmacy.
The health insurance company will send your patients a 16-digit activation code within a few working days, which they can use to register in the app and start treatment immediately.
The statutory health insurance companies cover the costs for Mindable in full. Patients do not have to make any advance or co-payments.
Private health insurants can apply for reimbursement, many private health insurances (PKV) also cover the costs for DiGA. However, there is no uniform regulation and it is up to the respective PKV to give this instead. Here it is advisable to inquire in advance directly with the respective PKV.

After successful registration, your patient:s will receive access to Mindable’s treatment program. This includes:
Psychoeducation: Users are first informed about the general mechanisms of anxiety and panic. Subsequently, maladaptive thoughts, safety and avoidance behaviors are identified and reflected upon through interactive exercises. The goal is to understand their perpetuating effect in order to actively reshape them in the next modules.
Symptom provocation (interoceptive exposure): Users are guided through animations to actively generate panic symptoms. Habituation desensitizes body memory and lowers the alertness of the central nervous system to panic stimuli. The goal is to reduce the number of panic attacks, as well as their severity.
Confrontation (in-vivo exposure): Users deliberately put themselves in fear-inducing situations through confrontation exercises. Here, too, habituation takes effect and conditioned stimulus-response patterns can be softened. More than 350 exercises are available for individualization, as well as the option to create your own exercises. During the exercise, a life recording of the anxiety level is made. Distorted perceptions can thus be corrected.
Checkup: enables weekly recording of symptom progress using targeted questions based on DSM-5 criteria. Progress in treatment becomes measurable and visible.
Diary: regular reflections in the diary serve to raise awareness of maladaptive patterns. Patients learn to relate panic attacks and symptoms to corresponding cognitions and behaviors.
Statistics & Histories: visualize the course of treatment. Patients are empowered in their self-efficacy and health literacy.

No. The prescription of Mindable and traditional psychotherapy are independent health insurance benefits. This does not affect the entitlement of your patients to psychotherapy.
Mindable is based on the evidence-based and S3 guideline-compliant methods of the treatment manual “Exposure-based therapy for panic disorder with agoraphobia” (Lang et al., 2018) and was developed in close collaboration with the authors. It is currently being evaluated in a prospective, multi-center, randomized control group trial (RCT) with symptomatic participants. Preclinical data already show significant changes.
